Health & Safety Form: Filming Day 1

Health and Safety form:

Locations and planned day of shoot	Shooting day is 2/12/13 All scenes today filmed at our school (Ruislip high). We will film down the sixth form corridor, the staff car park, the sixth form staircase, the sixth form common room
What equipment will be taken? Who will be present? What props are required?	Camera, note pad, teddy, two reading books. Present on the day will be Sam, Sophie and the two actors; Amy and James
What are the potential hazards associated with the location?	There are no hazards associated with this location.
Is there water at the location (pond, swimming pool, waterfall)? How will this be planned for?	There is no water at any of the locations
What are the risks associated with the general public? How will these be dealt with?	The only scene that we will film outside is the scene in the school car park so no members of the public will be around.
How will the equipment be stored during the shoot?	Everyone is responsible for each piece of  equipment they use in that scene, so they will carry it from different locations. On 2/12/13, Sophie is in charge of the camera for the night
How will the equipment be stored after the shoot/overnight?	In Sophie’s bedroom
